Catholic Scholar Nomination Form

Principals or teachers may submit this brief form to nominate 6th - 7th grade students to receive the Catholic Scholar Award from Chesterton Academy. This award recognizes students for their academic achievements.

To be eligible for this award, a student must:

Be nominated by a principal or teacher
Have 3.5 GPA or above
Demonstrate a high level of character and respect for teachers and peers

Each school may nominate one male and one female student in 6th grade and one male and female student in 7th grade per year. Students may only be nominated for this award once, so if two students were nominated as 6th graders, two additional members of their class may be nominated as 7th graders. The award will be applied to the recipient's tuition freshman year.

Please note, the Peacemaker Award and the Catholic Scholar Award cannot be awarded to the same student.
